What is NDIS Transition Support?

The aim of ADEC’s NDIS Transition Support Services is to assist people with disabilities, their carers, and their families, who are from culturally and linguistically diverse backgrounds, to transition to the NDIS.

Our focus is to build the capacity of NDIS participants and their family to confidently navigate the NDIS. Cultural awareness, Human rights and Self Advocacy.

Our workshop will be provided face to face with a presentation and questions and answers session to assist our community.

We will cover: *Understanding NDIS *Check Eligibility for NDIS

*Applying for NDIS *Submit Supporting Evidence (how and who to collect supporting documentation from)

*Pre-Planning

*Review Outcome and Progress

*Plan Implementation

*Receive an approved NDIS plan

*Attend a Planning meeting

*Schedule a Planning meeting

At the Workshop we will briefly touch on Appeals, Advocacy and Plan Management.
